Job Title: Certified Nursing Assistant (CNA) / Nursing Assistant – Long-Term Care

Job Summary: We are seeking a compassionate and dependable Certified Nursing Assistant (CNA) / Nursing Assistant to join our long-term care clinical team. In this role, you will deliver direct personal care and supportive nursing services to residents, assisting with activities of daily living (ADLs), mealtime support, mobility, and routine skin and vital monitoring to promote resident comfort, dignity, and safety.

Key Responsibilities:
  • Activities of Daily Living (ADLs): Assist residents with daily personal care routines, including bathing, dressing, grooming, toileting, hygiene, and personal care plans.
  • Mobility & Patient Transport: Safely assist residents with lifting, turning, positioning, and transferring into and out of beds, chairs, wheelchairs, and assistive mechanical lifts.
  • Dining & Nutritional Support: Coordinate dining services at mealtimes, including room setup, meal tray delivery, direct feeding assistance, delivery of nutritional supplements, and accurate documentation of food/fluid intake.
  • Clinical Observation & Reporting: Track resident skin condition (reporting pressure areas or skin breakdown immediately), obtain and record resident weights, and promptly notify nursing supervisors of changes in clinical condition.
  • Documentation & Compliance: Complete daily care flow sheets, document resident progress accurately, and maintain open communication with charge nurses and clinical managers.

Qualifications & Requirements:
  • Education: High School Diploma or GED Equivalent [Required].
  • Clinical Experience: Minimum 1+ year of recent nursing assistant experience in a Long-Term Care (LTC) or Skilled Nursing facility [Required].
  • Certifications & Licensure:
    • Active, unrestricted State Nursing Assistant Certification (CNA) with good standing [Required].
    • Active Basic Life Support (BLS) Certification through the American Heart Association (AHA) [Required].
  • Core Competencies: Strong empathy and patience with geriatric populations, physical stamina for resident transfers, reliable attendance, excellent observation skills, and effective documentation habits.
