Job Title: Director of Regional Care Management
Job Summary: We are seeking a strategic, extroverted, and innovative Director of Regional Care Management to provide interim leadership across a large, multi-facility healthcare system (600+ beds). In this critical role, you will hit the ground running to optimize workflows, consolidate departmental teams, and build immediate, strong relationships with physicians, care workers, and social workers to drive efficiency and meet organizational metrics.
Key Responsibilities:
- Leadership & Oversight: Direct and support a large team of 60+ FTEs—including care managers, intensive care coordinators, and social workers—across multiple hospitals and varying units.
- Workflow & Efficiency Optimization: Analyze current case management procedures to boost productivity, increase team visibility and morale, and drive overarching departmental efficiency.
- Care & Discharge Coordination: Decrease patient readmission rates, increase the completion of discharge planning, and actively address and escalate any systemic barriers to discharge.
- Strategic Collaboration: Bridge the gap between Behavioral and Medical social work managers to foster a unified team, and partner closely with revenue cycle and utilization management leaders.
- Compliance: Ensure all care management and social work operations strictly adhere to Federal and State healthcare guidelines and laws.
Qualifications & Requirements:
- Education: Bachelor of Science in Nursing (BSN), Licensed Independent Social Worker (LISW), or equivalent degree in healthcare management [Required]. Master’s degree in Nursing, Healthcare Administration, or Business [Preferred].
- Experience: Minimum 3 years (36 months) of recent Care/Case Management experience AND 3 years of progressive leadership/management experience [Required].
- Scale of Experience: Proven background overseeing a minimum of 10+ staff members within a large healthcare system (spanning multiple hospitals or 600+ beds) [Required].
- Licensure: Active Registered Nurse (RN) License or LISW equivalent, plus active Basic Life Support (BLS) certification [Required].
- Technical Skills: Proficiency in data analytics and Microsoft Tools (Excel, Power BI) [Required]; experience with EPIC EMR and Stratus finance systems [Highly Preferred].
