Job Title: Geriatric Nurse Practitioner (PACE Clinic)
Job Summary
We are seeking an experienced Nurse Practitioner (NP) for a high-impact role within a PACE Clinic (Program of All-Inclusive Care for the Elderly). This position focuses on providing comprehensive primary care to frail or elderly participants, helping them maintain their independence within the community. You will work autonomously while collaborating with a Medical Director to manage complex geriatric cases in an interdisciplinary environment.
Schedule & Shift Details
- Dates: June 15th through TBD (ongoing coverage).
- Shift: Monday – Friday, Day shift (8:00 AM – 5:00 PM).
- Setting: Outpatient PACE Clinic.
- EMR: EPIC.
Key Responsibilities
- Deliver primary health care and clinical management to a geriatric patient demographic.
- Perform initial and annual comprehensive health risk assessments.
- Develop and implement individualized care plans in collaboration with an interdisciplinary team.
- Coordinate with specialty providers and family members to ensure continuity of care.
- Utilize EPIC for all charting, documentation, and medical record management.
Required Qualifications
- Certification: Board Certified by ANCC or AANP [Required].
- Licensure: Active North Carolina State License [Required] (Will consider candidates willing to obtain).
- Experience: * Minimum 1 year of experience specifically working with the frail or elderly population [Required].
- Must be an Experienced Provider; no new graduates will be considered for this role.
- Credentials: Active BLS, CDS Certification, and DEA Registration [Required].
- Geography: Must be a local candidate. Only mileage (IRS standard rate) is reimbursed; no lodging or airfare provided.
