Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing / Long-Term Care
Job Summary: We are seeking a proactive and skilled Licensed Practical Nurse (LPN) to oversee daily unit clinical operations and deliver direct, high-quality patient care in a long-term care / skilled nursing facility setting. Working under the direction of the Nursing Supervisor and Director of Nursing, you will manage medication administration, complete treatments, supervise nursing assistants (LNAs/CNAs), carry out physician orders, and ensure a safe, regulatory-compliant environment.
Key Responsibilities:
- Direct Patient Care & Clinical Monitoring: Observe resident health status, receive and give comprehensive shift hand-off reports, and immediately notify supervisors and physicians of significant physical or mental changes.
- Medication & Treatment Administration: Safely administer medications and treatments using electronic MARs/TARs. Perform complex wound care (including wound vacs), administer oxygen therapy, and oversee G-tube/J-tube enteral feedings.
- Unit Leadership & Staff Supervision: Direct and delegate care to direct-care nursing assistants (LNAs/CNAs), ensuring adherence to scope of practice, infection control protocols, and unit cleanliness standards.
- Physician Orders & Rounds: Accurately transcribe and carry out physician orders. Accompany physicians on resident rounds to facilitate medical updates and 60-day care plan reviews.
- Admissions & Family Communication: Complete clinical documentation for admissions, discharges, and transfers. Communicate directly with family members regarding resident updates, care plans, and treatment consents.
- Controlled Substances & Compliance: Maintain strict compliance with state and federal regulations regarding the handling, storage, and change-of-shift counting of controlled substances.
Qualifications & Requirements:
- Experience: Minimum 3+ years of recent LPN nursing experience in a skilled nursing or long-term care setting [Required].
- Licensure: Active State LPN License or active Compact LPN equivalent [Required].
- Certifications:
- Active Basic Life Support (BLS) Certification through the American Heart Association (AHA) [Required].
- IV Therapy Certification (or willingness to obtain prior to start) [Required].
- Flexibility & Preferred Background: Willingness to float across clinical units as needed [Required]. Prior travel assignment experience and background in rural/small facility care [Highly Preferred].
