Job Title: Geriatric Nurse Practitioner (Permanent – PACE Clinic)
Job Summary
We are seeking a dedicated, highly experienced Nurse Practitioner (NP) for a permanent placement within a PACE Clinic (Program of All-Inclusive Care for the Elderly). In this vital role, you will collaborate closely with organization physicians and an interdisciplinary team (IDT) to deliver comprehensive primary care services to geriatric and frail populations. This role demands a clinician who can manage complex individualized care plans, perform acute illness evaluations, and conduct home visits when necessary to ensure the highest continuity of care.
Key Responsibilities:
- Primary & Acute Care: Deliver primary care services and evaluate/treat acute illnesses within the frail, elderly demographic.
- Comprehensive Assessments: Perform initial, ongoing, and as-needed participant health assessments.
- Care Planning & Management: Develop, implement, and manage individualized care plans while actively participating in interdisciplinary team (IDT) meetings.
- Transitions of Care: Manage discharge planning for hospital stays or long-term care transitions, conducting home visits as required.
- Clinical Oversight: Assist in the development of clinical policies and participate in the provider on-call rotation.
- Documentation: Ensure accurate, timely documentation of all clinical encounters.
Required Qualifications & Experience:
- Experience: Minimum 5–7 years of dedicated Nurse Practitioner experience [Required].
- Experience in long-term care or working with frail/elderly populations is strictly required. No new graduates will be considered.
- Board Certification: Board Certified NP via ANCC or AANP [Required].
- Licensure: Active, unrestricted Nurse Practitioner license in North Carolina [Required].
- Credentials: Active DEA Registration, CDS Certification, and BLS Certification [Required].
- History: Clean malpractice and license history is highly preferred.
- Citizenship: Must be eligible to work in the U.S. without sponsorship. Visa candidates (H1B/J1) will not be accepted.
