Job Title: Nurse Practitioner (NP) – Geriatric / PACE Primary Care
Job Summary:
We are seeking a dedicated, highly experienced Nurse Practitioner (NP) to join our interdisciplinary Program of All-Inclusive Care for the Elderly (PACE) team. In this role, you will deliver comprehensive, patient-centered primary care to frail and geriatric populations within a collaborative, mission-driven environment.
Working closely with staff physicians and an active Interdisciplinary Team (IDT), you will manage a unique panel of elderly participants, allowing you to develop, implement, and direct truly individualized care plans that maximize their independence and quality of life.
Key Responsibilities:
- Primary & Acute Care Delivery: Collaborate closely with team physicians to provide comprehensive primary care services, including evaluating and treating acute illnesses.
- Participant Assessments: Perform highly detailed initial, ongoing, and as-needed clinical assessments for fragile, complex geriatric patients.
- Interdisciplinary Collaboration: Participate actively in daily or weekly Interdisciplinary Team (IDT) meetings to synchronize medical, social, and therapeutic care.
- Continuity of Care: Manage discharge and transition planning for participants returning from hospital or long-term care stays; conduct home clinical visits when necessary to ensure strict care continuity.
- Clinical Documentation: Maintain accurate, thorough, and timely electronic documentation of all clinical encounters.
- On-Call Rotation: Share responsibility in the provider on-call rotation schedule.
- Policy Development: Support the clinical leadership team by assisting in the development and implementation of progressive clinical policies and procedures.
Candidate Requirements & Qualifications:
Education & Experience:
- Clinical Experience: Minimum of 5 to 7 years of active Nurse Practitioner experience [Required]. Due to the high-acuity needs of our participants, new or recent graduates cannot be considered.
- Specialized Expertise: Direct experience working extensively with frail or elderly populations is [Required] (or a highly demonstrated willingness to undergo rigorous geriatric training). Background in long-term care or skilled nursing settings is highly preferred.
- Background Standing: A clean medical malpractice and licensure history is [Highly Preferred].
Licensure, Boards & Certifications:
- Board Certification: Active National Board Certification through the ANCC or AANP [Required].
- State Licensure: Active, unrestricted Nurse Practitioner license in the State of North Carolina [Required].
- Prescriptive Authority: Active Federal DEA Registration and state-level Controlled Substance (CDS/CSR) Certification [Required].
- Life Support: Active Basic Life Support (BLS) certification [Required].
