Job Title: Healthcare Care Manager (RN) or Medical Social Worker (BSW)

Job Summary:
We are seeking dedicated healthcare professionals to join our patient care team as either a Care Manager (RN) or a Medical Social Worker (BSW). In these vital roles, you will coordinate individualized plans of care focusing on quality, timely service delivery, and the clinical necessity of patient admissions.

Whether you are an experienced Registered Nurse or a licensed Social Worker, you will assess, plan, implement, monitor, and evaluate clinical options to meet the unique healthcare needs of our patients and their supporting family systems. We are looking for collaborative professionals who maintain an open, approachable, and respectful manner, ensuring well-orchestrated care centered entirely on patient quality, efficiency, and safety.

Compensation & Shift Details:
  • Care Manager (RN) Hourly Rate: Starting at $30.11+ / hour (Dependent on education and experience)
  • Social Worker (BSW) Hourly Rate: Starting at $24.77+ / hour (Dependent on education and experience)
  • Relocation Assistance: Up to $5,000 in relocation allowance is available (to be discussed further during the interview process).

Key Responsibilities:
  • Care Coordination: Direct the development and execution of patient care plans, ensuring the appropriate level of care and medical necessity upon admission.
  • Holistic Assessment: Evaluate, implement, and monitor a diverse range of clinical services to meet the physical and emotional healthcare needs of patients and their families.
  • Patient Advocacy: Maintain an open, compassionate, and approachable communication style that preserves patient dignity, self-confidence, and mutual respect.
  • Interdisciplinary Collaboration: Partner closely with physicians, nursing staff, and auxiliary teams to drive positive health outcomes through well-orchestrated care transitions.

Candidate Requirements & Qualifications:
Option 1: Care Manager (RN) Tracks:
  • Education: Associate's Degree in Nursing (ADN) or a Diploma Degree from an accredited registered nurse program [Required]; Bachelor of Science in Nursing (BSN) [Highly Preferred].
  • Experience: Minimum of 2 years of acute care medical/surgical nursing experience [Required].
  • Licensure: Current, active Registered Nurse (RN) license for the State of Kansas [Required].
Option 2: Social Worker (BSW) Tracks:
  • Education: Bachelor’s Degree in Social Work or closely related social services field [Required].
  • Experience: At least 2 years of hospital-based social work experience [Highly Preferred].
  • Licensure: Current, active Licensed Bachelor of Social Work (LBSW) credential for the State of Kansas [Required].
