🩺 Key Responsibilities:
• Provide primary care services in collaboration with supervising physicians
• Perform routine, annual, and as-needed patient assessments
• Manage chronic conditions and acute illnesses
• Develop individualized care plans within the interdisciplinary team
• Coordinate referrals and transitions of care
• Educate patients, families, and caregivers on treatment plans
• Maintain compliant clinical documentation
• Participate in provider on-call rotation
📋 Requirements:
• Graduate of an accredited PA program
• Active North Carolina PA License or willingness to obtain
• Minimum 2 years PA experience required
• Geriatric or frail elderly patient experience preferred
• BLS Certification required
• DEA Registration required
• Strong independent clinical decision-making skills
• NPDB Self Query Report within 30 days required
