Job Title: Community Health / Home Health Registered Nurse (RN).
Job Summary:
We are seeking an autonomous, highly skilled Community Health / Home Health Registered Nurse (RN) to deliver comprehensive, compassionate nursing care directly to patients in their homes. In this role, you will perform clinical assessments, establish care plans, administer treatments, and educate families to promote healing, manage chronic conditions, and prevent hospitalization.
Operating under general guidance, this position requires an exceptional level of critical thinking, clinical independence, and sharp time-management skills to efficiently self-schedule and manage an active field caseload.
Key Responsibilities:
- In-Home Clinical Care: Provide, supervise, and document advanced nursing care for sick and injured patients within a home health framework, performing complex assessments and clinical interventions.
- OASIS Documentation: Complete accurate, timely, and fully compliant OASIS (Outcome and Assessment Information Set) documentation to align with home care reimbursement and regulatory guidelines.
- Care Coordination: Collaborate actively with physicians, physical therapists, social workers, and community representatives to build a cohesive, interdisciplinary continuum of care.
- Patient & Family Education: Teach patients and designated family caregivers how to safely manage medication regimens, recognize adverse symptoms, and maintain a safe home environment.
- Caseload Management: Independently manage daily scheduling, prioritize urgent patient visits, and adapt efficiently to changing clinical needs across your assigned territory.
Candidate Requirements & Qualifications.
Experience & Clinical Background:
- Home Health Focus: Proven experience working in a dedicated home health environment [Required].
- Regulatory Competency: Certified in Home Health protocols with verified proficiency in handling OASIS assessments [Required].
- Acute Care Baseline: Minimum of one (1) year of acute clinical nursing experience, with hospital-based experience highly preferred.
- Specialized Skills: Infusion Therapy Certification is a plus [Preferred].
- Education: Bachelor of Science in Nursing (BSN) is highly preferred.
Licensure, Credentials & Logistics:
- RN Licensure: Active, unrestricted State Registered Nurse (RN) license in hand [Required at time of application].
- Life Support: Active Basic Life Support (BLS) certification through the American Heart Association (AHA) [Required].
- Transportation & Driving: Must possess a valid Driver’s License, active auto insurance, and a reliable personal vehicle. Candidates must be fully comfortable driving in inclement weather conditions to reach patients.

- Demonstrated ability to perform independently as a clinician with minimal direct supervision.
- Strong history of dependability in meeting committed schedules and maintaining emotional stability during emergency interventions.
- Flexibility: Willingness to maintain flexible work hours and participate in the department's "on-call" rotation as required.
Strict Facility Travel Policies:
- Radius Restriction: To qualify for this contract traveler position, your permanent, legal residence must be greater than 50 miles away from the facility. Permanent address verification must be provided up front.
- Planned Time Off (RTO): A maximum of five (5) days of requested time off can be considered for this assignment. All RTO dates must be confirmed at the time of your initial application; scheduling adjustments cannot be accommodated during the interview or after an offer is extended.
