Job Summary: We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to prescribe, coordinate, and deliver high-quality nursing care. In this role, you will perform comprehensive client assessments, develop and modify individualized Plans of Care, complete OASIS documentation, and empower patients and families through health education, all while maintaining strict standards of safety and privacy.
Key Responsibilities:
- Patient Assessment & Care: Conduct comprehensive initial and ongoing physical, psychosocial, and environmental assessments across the lifespan; deliver direct clinical nursing care in accordance with healthcare regulations.
- Care Planning & Discharge: Establish, re-evaluate, and modify Plans of Care in collaboration with patients, physicians, and caregivers; develop effective discharge plans.
- Documentation & Compliance: Complete accurate OASIS data collection and maintain thorough, timely clinical documentation in line with regulatory guidelines.
- Education & Support: Assess the learning needs of patients and family caregivers; implement appropriate teaching strategies to promote optimal health outcomes.
- Supervision & Safety: Direct and supervise Certified Nursing Assistants (CNAs); coordinate care with interdisciplinary team members while maintaining a clean, safe care environment and upholding patient privacy and confidentiality.
Qualifications & Requirements:
- Experience: Demonstrated experience in Home Health care [Required] (candidates with strong Medical-Surgical experience combined with some Home Health exposure may also be considered).
- Documentation Skills: Prior OASIS and Start of Care (SOC) experience [Strongly Preferred].
- Licensure: Active Registered Nurse (RN) License [Required].
- Certifications: Basic Life Support (BLS) certification [Required].
- Transportation: Valid driver’s license and active auto insurance [Required].
- Immunizations: Up-to-date COVID-19 and Influenza vaccinations [Required].
