Job Title: Hospice Registered Nurse (Hospice Case Manager)
Job Summary: We are seeking an experienced and compassionate Hospice Registered Nurse (RN) to manage a dedicated caseload of end-of-life patients. In this role, you will deliver comprehensive nursing care across various service levels, manage patient visits, coordinate care plans with families and interdisciplinary teams, and complete daily clinical charting to ensure comfort and quality of life for patients and their loved ones.
Key Responsibilities:
- Caseload Management: Oversee a caseload of approximately 15 hospice patients, conducting 4 to 5 visits per day based on geographic location and patient acuity levels.
- Comprehensive Hospice Nursing: Provide all service-level nursing care, including Routine, General Inpatient (GIP), Continuous, and Respite care.
- Visits & Admissions: Conduct initial admissions, Service Intensity Add-on (SIA) visits, and end-of-life/death visits with empathy and clinical excellence.
- Clinical Documentation: Complete all electronic charting and patient record updates on the same day as visits.
- Patient & Family Support: Educate caregivers on symptom management, medication administration, and emotional support throughout the hospice process.
Qualifications & Requirements:
- Clinical Experience: Minimum of 1+ years of current Hospice Case Management experience [Required]. (Acute Care / Med-Surg experience is highly preferred).
- Licensure: Active Registered Nurse (RN) License [Required].
- Certifications: Active Basic Life Support (BLS) certification through the American Heart Association (AHA) [Required].
- Transportation & Driving Record: Active Driver’s License, valid auto insurance, and a reliable personal vehicle to cover up to a 60-mile service radius [Required]. (Travel mileage is reimbursed at standard IRS rates).
- Compliance: Ability to pass standard background screening, drug screen, and provide required immunization records (TB, Influenza, COVID-19).
