Job Title: Hospice On-Call Nurse (RN / LPN)

Job Summary

We are seeking a compassionate, responsive Hospice On-Call Nurse to join our dedicated end-of-life care team. Operating from our regional hub in Christiansburg, Virginia, you will be responsible for managing after-hours patient needs, fielding triage calls, and making urgent home or facility clinical visits when requested. The ideal candidate will blend strong critical-thinking skills with a deep capacity to provide emotional and educational support to patients and their families during sensitive moments.

Key Responsibilities

  • Responsive Patient Care: Appropriately handle incoming patient and facility requests after hours, providing triage phone guidance or deploying for immediate in-person visits when necessary.
  • Family & Patient Support: Deliver vital clinical education, symptom management guidance, and compassionate emotional support to terminal patients and their loved ones.
  • Electronic Documentation: Seamlessly document all clinical assessments, phone triages, and home interventions electronically; ensure a comprehensive activity hand-off is transmitted electronically immediately at the end of each shift.

Candidate Requirements & Qualifications

Licensure & Credentials

  • State Licensure: Active nursing license recognized by the State of Virginia [Required]. (Multi-state / Compact licenses are allowed and welcomed).
  • Driver's License: Valid driver's license with a clean, reputable driving record [Required].

Experience & Logistics

  • Hospice Background: A minimum of 1+ year of dedicated hospice or palliative care experience [Required].
  • Transportation: Must possess highly reliable personal transportation to safely navigate to home and facility visits during on-call hours [Required].

Core Competencies

  • Strong clinical triage skills and the ability to work completely autonomously during off-hours.
  • Proficient with Electronic Medical Record (EMR) tracking software for immediate, accurate remote charting.
  • Exceptional communication and crisis-intervention skills.
