We are actively seeking an experienced Licensed Practical Nurse (LPN/LVN) for a travel contract opportunity at a long-term care facility in Claremont, New Hampshire.

Shift Options:
- Evening Shift: 3:00 PM – 11:00 PM
- Weekend Rotation Shift: 3:00 PM – 7:00 AM
Schedule:
- 40 hours per week.
- Every other weekend required.

Position Overview:
Under the supervision of the Nursing Supervisor and Director of Nursing, the LPN/LVN will oversee assigned residents and provide high-quality nursing care while ensuring compliance with facility standards, state regulations, and patient safety protocols.
This role requires strong clinical judgment, adaptability, teamwork, and excellent communication skills.
Key Responsibilities:
Resident Care & Monitoring:
- Observe residents for changes in physical and mental condition.
- Communicate clinical concerns appropriately to supervisors and physicians.
- Manage detailed shift-to-shift reporting and documentation.
Medication Administration & Treatments:
- Administer medications and treatments accurately using electronic MAR/TAR systems.
- Receive and transcribe physician orders.
- Provide specialized nursing care including:
- Oxygen administration.
- Gastrostomy/jejunostomy feedings.
- Advanced wound care and wound vac management.
Admissions, Transfers & Documentation:
- Coordinate resident admissions, discharges, and transfers.
- Ensure all required documentation and notifications are completed accurately.
Leadership & Team Support:
- Supervise and guide Licensed Nursing Assistants (LNAs).
- Assign and monitor care duties.
- Support onboarding and mentorship of nursing staff.
- Assist with maintaining infection control and unit safety standards.
Requirements:
Experience:
- Minimum 3 years of current clinical experience REQUIRED.
- Active clinical practice within the last 6 months REQUIRED.
- Prior travel nursing experience strongly preferred.
- Experience in rural or smaller healthcare facilities preferred.
- Must be comfortable floating to units where experienced.
Licensure & Certifications:
- Active New Hampshire or Compact LPN License REQUIRED.
- IV Certification required or willingness to obtain during onboarding.
- Active BLS Certification (AHA) REQUIRED.
- COVID-19 vaccination REQUIRED.
